Utilidad pronóstica del dímero d en pacientes con covid-19 y diabetes mellitus subyacente

Resumen

Los pacientes diabéticos son susceptibles a resultados adversos y complicaciones durante la infección por Coronavirus. La coexistencia de estas dos enfermedades estan asociadas con mayor severidad, por lo que se necesitan de biomarcadores que permiten predecir estados de mayor severidad y mejoren la mortalidad observada durante la pandemia. El objetivo es analizar evidencias sobre la utilidad pronóstica del dímero D en pacientes con COVID-19 y diabetes mellitus subyacente, la metodología es de diseño documental de tipo descriptivo, exploratorio y de alcance explicativo. Con búsqueda de artículos en las bases de datos PubMed, SciELO, Elsevier, Redalyc, Web of Science y Cochrane Library, publicados entre los años 2018-2022. Se seleccionaron 113 artículos bajo criterios de inclusión y exclusión. Los estudios evidenciaron alta prevalencia global del 53,4% de pacientes diabéticos con COVID-19 y una tasa de mortalidad del 54,9% en relación a todos los estudios. Al comparar las concentraciones de dimero D en pacientes con COVID-19 con y sin diabetes mellitus, se observó un rango hipervariable tanto en los pacientes con diabetes (0,13 mg/L a 5,075 mg/L), como sin diabetes (0,09 a 4,62 mg/L), pero asociadas a mayor severidad y gravedad de la enfermedad y al requerimiento de cuidados críticos en estos pacientes.
